Please bring the completed New Patient Information Sheet and Questionnaire with you, along with any x-rays, CT or MRI films and reports you’ve had taken pertaining to this problem. In addition, please bring your insurance cards, a photo ID, and if you have a co-pay come prepared to provide the amount prior to your visit. Our office accepts cash and checks, as well as cards through Visa, Discover, and Mastercard.
Please bring a list of your medications, including dosages, or bring your medicine bottles in a plastic bag. Also remember to bring any stent/implant cards to be scanned into your chart.
If we are evaluating a knee problem, please bring or wear a pair of shorts. If you are a female and we are evaluating a shoulder problem, please bring or wear a tank top if you have one.
If you need to cancel an appointment we require one business day's notice or there will be a $25.00 fee. When we do not receive sufficient notice we cannot offer these spots to those who need them.
If you are more than 20 minutes late for your appointment you may be rescheduled. Please help us to stay as close to our schedule as possible so we can provide each patient the necessary time they need.
By printing and filling out the New Patient Form and relevant Questionnaire prior to your appointment, you can save time at our office (these forms are in PDF format).
